Patient’s Bill of Rights and Responsibilities

للمريض الحق في

1. أن يكون على علم بحقوقه ومسؤولياته.
2. الحصول على معلومات عن الخدمات المقدمة في المستشفى.
3. توفير بيئة آمنة له في جميع مرافق المستشفى.
4. معرفته بهوية الأطباء أو غيرهم من الممارسين أو الأخصائيين المسؤولين عن رعايته.
5. معاملة باحترام وكرامة في جميع الأوقات وتحت جميع الظروف.
6. احترام عاداته وتقاليده وقيمه ومعتقداته الدينية (شريطة أن لا تتعارض مع الشريعة الإسلامية).
7. احترام خصوصيته عند تقديم الرعاية الطبية حسب احتياجاته.
8. طلب وجود محرم أو من يرغب من مرافقيه أو أحد أفراد الطاقم الصحي من نفس الجنس أثناء الفحص الطبي أو العلاج.
9. الحفاظ على سرية جميع المعلومات والسجلات المتعلقة برعايته في المستشفى.
10. حمايته من الاعتداء الجسدي واللفظي.
11. تقييم وإدارة الألم المناسبة الناتجة عن حالته الصحية.
12. المشاركة فيما يتعلق برعايته الصحية، بما في ذلك المشاركة في اتخاذ القرارات الخاصة بالرعاية، والسؤال عن كل ما يتعلق برعايته والحصول على رأي آخر دون خوف من التأثير السلبي على الرعاية المقدمة له.
13. توعيته بكل ما يتعلق بحالته الصحية.
14. أخذ موافقته الخطية بعد حصوله على المعلومات الكافية قبل الجراحة أو التخدير أو استخدام الدم ومشتقاته، أو أي إجراء عالي الخطورة.
15. رفض أو وقف العلاج وتزويده بالمعلومات الكافية عن العواقب المترتبة على قراره والمسؤوليات المتعلقة بهذا القرار والعلاج البديل.
16. حصوله على الرعاية الكريمة والاحترام في حالة الاحتضار والوفاة.
17. أخذ موافقته الخطية قبل المشاركة في الأبحاث العلمية والتجارب الطبية، وله الحق في رفض المشاركة أو الانسحاب في أي وقت دون التأثير على رعايته في المستشفى.
18. الحصول على معلومات عن كيفية التبرع وزراعة الأعضاء والأنسجة الأخرى وفقاً لسياسة المستشفى.
19. تقديم الشكوى فيما يتعلق بحالته الصحية أو الخدمات المقدمة والتحقيق فيها وحلها حسب إمكانيات المستشفى.
20. حماية ممتلكاته عندما يكون غير قادر على ذلك أو عندما ينعهد المستشفى بتحمل مسؤولية حمايتها.
21. حصوله على تقرير طبي عن حالته الصحية وفقاً للقواعد والأنظمة المعمول بها في المستشفى.

المريض مسؤول عن

1. اتباع قوانين ولوائح المستشفى.
2. تقديم المعلومات الدقيقة والكاملة للطاقم الطبي فيما يتعلق بتاريخه المرضي السابق.
3. تبليغ الطاقم الطبي المسؤول عن رعايته في حالة عدم فهم المعلومات المتعلقة برعايته أو عدم قدرته على الالتزام بالتعليمات الطبية.
4. الالتزام بالمواعيد أو الاتصال لإلغائها عندما لا يستطيع الحضور للمواعيد الخارجية أو التنويم.
5. الالتزام بسياسة حظر التدخين في مرافق المستشفى.
6. اتباع تعليمات الطاقم الطبي والالتزام بخطة العلاج وتحمل مسؤولية الرفض في حالة رفض العلاج أو رفض اتباع التعليمات الطبية.
7. مراعاة حقوق المرضى الآخرين والمحافظة على ممتلكات المستشفى.
8. التعامل مع موظفي المستشفى بلطف واحترام.
9. إبلاغ المستشفى في حالة أي تغيير في الحالة العلاجية.
10. مغادرة المستشفى مباشرة وبالخروج متى ما أمر الطبيب المعالج.

The Patient has the right to

1. Be informed about their rights and responsibilities.
2. Be informed of the services provided in the hospital.
3. Receive care in a safe environment.
4. Know the identities of the physicians and other practitioners responsible for his/her care.
5. Be treated with respect and dignity at all times and under all circumstances.
6. Receive care that is respectful of their values and beliefs (provided that they don’t conflict with Islamic rules).
7. Respect and maintain their privacy during care and according to their needs.
8. Have a significant other, chaperone or a staff member from the same gender be present during physical examination, treatment or procedure.
9. Maintain confidentiality of all information, communication and records related to the care.
10. Be protected from physical and verbal assault.
11. Appropriate assessment and management of pain.
12. Participate in their care process including making decisions or asking questions about care, and seeking a second opinion without fear of compromising care.
13. Be educated about their condition.
14. Consent to surgery, high-risk treatments and procedures, anesthesia, and use of blood and blood products, after receiving adequate information.
15. Refuse or discontinue treatment and be informed of the consequences of the decision, responsibilities related to such decision and available care and treatment alternatives.
16. Respectful and compassionate care at the end of life.
17. Be fully informed about clinical research which they agree to participate in, and the right to refuse or withdraw consent without affecting the care provided.
18. Be informed about donation and transplantation of organs and other tissues, as per hospital policy.
19. Voice a complaint about care or hospital services, and have the complaint investigated and resolved whenever possible.
20. Have their possessions protected once the hospital assumes responsibility for protecting possessions or when the patient cannot assume responsibility.
21. Receive a report on their medical condition according to hospital regulations.

The Patient is responsible to

1. Follow the hospitals rules and regulations.
2. Provide accurate and complete information about his/her past medical history and current medical condition.
3. Inform his/her care provider when he/she does not understand information related to care.
4. Keep outpatient and admission appointments, or call to cancel if unable to attend.
5. Follow the hospital no smoking policy.
6. Follow his/her care providers’ instructions, and accept responsibility for his/her actions when refusing treatment or not following the care providers’ instructions.
7. Be considerate of other patients and the hospital’s property.
8. Show courtesy and respect to the hospital personnel.
9. Inform the hospital about changes in his/her eligibility condition.
10. Must leave the hospital without delay when discharged.
